Massage Therapy Services are a covered benefit of many health insurance plans in Washington State. I am able to bill your insurance company, but there are a few things you should know first.
- Many insurance companies require a prescription from a doctor/chiropractor/physical therapist in order to cover massage therapy services.
- Some insurance companies require that massage practitioners be "preferred providers" in order to pay for services. Currently, I am a preferred provider for:

I can bill other insurance companies as an "out of network" provider.
- Many insurance companies require that your use of massage therapy is for a specific medical condition that will improve within a short amount of time. As soon as improvement slows down or halts, insurance companies deem this "wellness" or "maintenance" massage and will no longer cover services.
- I am able to bill Labor & Industries claims as well as Person Injury Claims (auto insurance).
If you would like to have me bill your insurance company, please print the billing information form and complete it. You will need to contact your insurance provider to gather the necessary information. This form will enable us to determine if your insurance will pay for massage services and if there are any limits to those services.
Please also have your health care provider fill out a prescription form to verify that massage services are medically necessary.
